Define the six-task plan for lifecycle evidence, boundary-confirmed attribution, adapter identity and classification, durable recovery, plugin wiring, and end-to-end regressions for OpenCode mutation scopes. Preserve the existing generic ingress, provenance semantics, and conservative false-positive guard. Plan: opencode-mutation-scope-integration Tasks: T01, T02, T03, T04, T05, T06
