Scope:
Cover start-issue supervisor launch, polling, review/fix/re-review and cleanup simulation.

Acceptance:

• Implement the feature according to the feature brief.
• Cover the use case with an e2e test that executes the CLI/supervisor entrypoint, not only internal functions.
• Keep JSON output agent-first and stable for automation.
